Before you fax

• Pick the number by where you live: Milwaukee County uses the MDPU, every other county uses the CDPU.
• You can also send proof through your ACCESS account at access.wi.gov or the MyACCESS app.
• To mail instead: CDPU, PO Box 5234, Janesville, WI 53547-5234, or MDPU, PO Box 05676, Milwaukee, WI 53205.
• Write your name and case number on each page so it reaches your case.

How to send it without a fax machine

1. Gather the proof your notice asks for.
2. Scan or photograph every page, with your name and case number on each.
3. Enter 855-293-1822, or 888-409-1979 in Milwaukee County, and add the pages.
4. Send, and keep the delivery receipt.
