Before you fax

• Put your name, DCN or Social Security number, and date of birth on every page or document.
• You can also upload documents at mydssupload.mo.gov, from a phone, tablet or computer.
• Or take them to your local resource center during office hours.

How to send it without a fax machine

1. Write your name, DCN or SSN, and date of birth on each page.
2. Scan or photograph the pages.
3. Enter 573-526-9400 and add them.
4. Send, and keep the delivery receipt.
