Before you fax

• You can also email documents to doh.dpa.info@alaska.gov; include your name and date of birth if you have an open case.
• For questions, the Virtual Contact Center is 800-478-7778, Monday to Friday 8 a.m. to 5 p.m.
• Write your name and case number on each page so it reaches your case.

How to send it without a fax machine

1. Gather the application or proofs (pay stubs, rent) you were asked for.
2. Scan or photograph every page.
3. Enter 888-269-6520 and add the pages.
4. Send, and keep the delivery receipt.
