• For questions, call 866-435-7414 toll-free or 801-526-0950 in the Salt Lake area, Monday to Friday 8 a.m. to 5 p.m.
• Write your name and case number on each page so it reaches your case.

How to send it without a fax machine

1. Gather the paperwork DWS asked for.
2. Scan or photograph every page, with your name and case number on each.
3. Enter 877-313-4717 and add the pages.
4. Send, and keep the delivery receipt.
