Fax documents to DTA in Massachusetts

Before you fax

• Include the DTA cover sheet. DTA says documents sent without it can be delayed in processing.
• You can upload documents instead with DTA Connect, DTA's free app and website (DTAConnect.com), and see there whether documents you sent have been processed.
• To mail them, send the documents and cover sheet to DTA Document Processing Center, PO Box 4406, Taunton, MA 02780.

How to send it without a fax machine

1. Fill in the DTA cover sheet and put it on top of your documents.
2. Scan or photograph every page, cover sheet first.
3. Enter 617-887-8765 and add the pages.
4. Send, and keep the delivery receipt as proof of when you faxed it.
