Before you fax

• To mail forms instead: Human and Community Services, PO Box 202925, Helena, MT 59620.
• For questions, call the Public Assistance Helpline at 888-706-1535 (TTY 711).
• Write your name and case number on each page so it reaches your case.

How to send it without a fax machine

1. Fill in and sign the form, or gather the documents you were asked for.
2. Scan or photograph every page.
3. Enter 877-418-4533 and add the pages.
4. Send, and keep the delivery receipt.
