Before you fax

  • The IRS prefers Form 14039 online, at IRS.gov/dmaf/form/f14039, though attachments can't be added there.
  • Choose one way to send it, online, mail or fax, not several, and don't send another Form 14039 for an incident you already reported.
  • Always include a cover sheet marked "Confidential".
  • If you can't e-file because someone misused your SSN or ITIN, attach Form 14039 to your paper return and send both where you normally file, not to this number.
  • Never fax an original tax return to this number.

Can I send my tax return to this number?

No. The IRS says not to use this fax number or address for an original tax return.

Does the number change?

It can. We compare it with the current Form 14039 regularly; the date we last checked is at the bottom of this page.
