Fax a power of attorney to the Minnesota Department of Revenue

Before you fax

• The same number takes the department's other authorization forms, and the REV184r to remove an authority.
• Minnesota also accepts federal Form 2848 for representation or receiving information, if a Minnesota tax type or form is listed in section 3.
• You can also email the forms to MNDOR.POA@state.mn.us.
• To mail: Minnesota Department of Revenue, Mail Station 4123, 600 N. Robert St., St. Paul, MN 55146-4123 (Forms M100i, M100b and M100e go to Mail Station 7703, St. Paul, MN 55146-7703).

How to send it without a fax machine

1. Complete and sign the form.
2. Scan or photograph every page.
3. Enter 651-556-5210 and add the pages.
4. Send, and keep the delivery receipt.
