Before you fax

  • Fill out the form legibly, then sign and date it. Keep a copy.
  • Send it by fax or by mail, but not both. To mail: NYS Tax Department, POA Central Unit, W A Harriman Campus, Albany NY 12227-0864.
  • With an Online Services account you can file the POA electronically instead. A POA made with the Department's web application can also be printed, signed and faxed.
  • The Department does not accept IRS Form 2848 or 8821, or another state's or city's POA form (such as New York City Form POA-2), in place of Form POA-1.
  • For child support enforcement matters, call 518-485-6820 first for instructions.

How to send it without a fax machine

  1. Complete Form POA-1, then sign and date it.
  2. Scan or photograph every page.
  3. Enter 518-435-8617 and add the pages.
  4. Send, and keep the delivery receipt with your copy.
