Before you fax

• All signatures must be handwritten. Electronic or stamped signatures are not accepted.
• You can only appoint a person as representative, not a business: your tax preparer, but not the preparer's office.
• Allow up to 2 weeks for processing.
• Form R-7, to enroll as a Virginia authorized agent, goes to the same fax number.

1. Complete Form PAR 101 and sign it by hand.
2. Scan or photograph every page.
3. Enter 804-254-6115 and add the pages.
4. Send, and keep the delivery receipt.
