Before you fax

  • Each number is printed on its own Humana form. Humana's exceptions and appeals page says to send the form to the address or fax number on the form, and it does not print the numbers itself, so use the form that matches what you are appealing.
  • Humana also takes appeals and grievances online, and by phone at the Customer Care number on the back of your member ID card.
  • Mail goes to Humana Grievance and Appeals Department, P.O. Box 14165, Lexington, KY 40512-4165. For a Humana employer plan, Humana's page says to mail the form to P.O. Box 14546, Lexington, KY 40512-4546, or call the Customer Care number on your ID card; it doesn't list fax for employer plans.
  • For a Medicare Advantage (Part C) appeal you have 65 days from the date of the denial notice; the same 65 days applies to a Part D prescription drug redetermination. Check your denial letter for your own deadline.
  • An expedited appeal is only for services you have not yet received, and Humana asks for a statement from your provider. Humana takes expedited appeals by fax for Part D (prescription drugs) only. For an expedited medical (Part C) appeal, call 800-867-6601 or mail the form to the Humana Expedited Appeals Unit, P.O. Box 14165, Lexington, KY 40512-4165.
  • Humana's Part D redetermination form lists 800-451-4651 for expedited drug appeals by phone, and an online option at humana.com/denial.
  • If someone else files for the member, Humana needs a valid authorization on file, such as its Appointment of Authorized Representative form, or a court order or other legal document.

How to send it without a fax machine

  1. Pick the form: the Request for Redetermination form for a Part D prescription drug denial, the Appeal or Grievance Form for everything else. Fill it in and sign it.
  2. Tick the expedited box only for a Part D drug appeal that can't wait. For an urgent medical appeal, call 800-867-6601 instead of faxing.
  3. Scan or photograph every page, plus receipts, medical records or a letter from your provider.
  4. Enter 877-556-7005 for the Part D redetermination form, or 888-556-2128 for the Appeal or Grievance Form, and add the pages.
  5. Send, and keep the delivery receipt as proof of the date you sent it.
