Before you fax

  • Put your FEMA application number and the disaster number on every page.
  • Add a cover letter marked "Attention: FEMA - Individuals & Households Program" that lists the documents you are sending.
  • An appeal is due within 60 days of the date on your FEMA decision letter.
  • If someone else appeals for you, include a statement signed by you that authorizes them, unless FEMA already has one.
  • Other ways to send: upload at DisasterAssistance.gov or in the FEMA app, mail to FEMA - Individuals & Households Program, National Processing Service Center, P.O. Box 10055, Hyattsville, MD 20782-8055, or bring them to a Disaster Recovery Center.
