Before you fax

  • Check the tables on page 3 of the SF-180: they say where the record is kept, by branch and separation date. Most are at the NPRC in St. Louis.
  • Sign and date the request. Federal law requires written requests for non-archival records to be signed and dated within the last year.
  • Send a separate SF-180 for each veteran; one fax can carry several.
  • For an emergency, write it in the "Purpose" section and fax to 314-801-0764. For a burial outside a National Cemetery, include the next of kin's signature and proof of death.
  • Don't send original documents: anything sent with the request is not returned.
  • You can also order online with eVetRecs, or mail the form to National Personnel Records Center, 1 Archives Drive, St. Louis, MO 63138.

  3. Enter 314-801-9195, or 314-801-0764 for an emergency, and add the pages.
  4. Send, and keep the delivery receipt.
