• Customer Service phone: 651-296-3644 in the Twin Cities area, 877-898-9090 in Greater Minnesota.
• Write your name and the last four digits of your Social Security number on each page so it reaches your account.

How to send it without a fax machine

1. Gather the pages you were asked to send.
2. Scan or photograph every page.
3. Enter 651-205-4007 and add the pages.
4. Send, and keep the delivery receipt.
