Before you fax

  • Often a claim investigator gives you a link to upload documents instead; use it if you have one.
  • To mail instead: Indiana Department of Workforce Development, Indiana Government Center South, 10 North Senate Avenue, Indianapolis, IN 46204.
  • Some DWD forms print their own fax number; if yours does, use that one.
  • Benefit Call Center: 800-891-6499.
  • Write your name and claimant ID on each page so it reaches your claim.

How to send it without a fax machine

  1. Gather the documents DWD needs for your claim.
  2. Scan or photograph every page.
  3. Enter 317-633-7206 and add the pages.
  4. Send, and keep the delivery receipt.
