# Fax unemployment documents to Utah's Department of Workforce Services

Utah Unemployment Insurance has three fax numbers: 801-526-4401 for claims, 801-526-9394 for monetary (wage) issues and 801-526-4402 for adjudication, which also takes the eligibility forms.

## Before you fax

- Send forms the way the department told you to; if your instructions give a fax number, use it.
- To mail eligibility forms instead: Unemployment Insurance, PO Box 45266, Salt Lake City, Utah 84145-0266.
- For questions, call the Claims Assistance and Re-Employment Team (801-526-4400 in Salt Lake and South Davis counties, 888-848-0688 for the rest of the state and out of state).
- Write your name and claimant ID on each page so it reaches your claim.

2. Scan or photograph every page.
3. Enter the number and add the pages.
4. Send, and keep the delivery receipt.
