# Fax a SNAP application or documents to NYC HRA

In New York City, fax a new SNAP application to HRA at 917-639-1111, and a recertification, or documents for an application you already sent, to 917-639-2544.

## Before you fax

- HRA says the quickest way to apply or recertify is online with ACCESS HRA.
- The application form is LDSS-4826, available from HRA in several languages.
- To mail it instead: Supplemental Nutrition Assistance Program, P.O. Box 29008, Brooklyn, NY 11202.
- Outside New York City, SNAP applications go to your county's department of social services, not to these numbers.

## Steps

1. Fill in and sign the SNAP application (LDSS-4826) and gather the documents it asks for.
2. Scan or photograph everything.
3. Enter 917-639-1111 for a new application, or 917-639-2544 for a recertification or later documents.
4. Send, and keep the delivery receipt.
