# Fax an SF-180 to request military records or a DD-214

## Fax numbers

| Who | Fax number |
| --- | --- |
| Routine requests for military personnel records, including the DD-214 | 314-801-9195 |
| Emergency requests, and burials outside a National Cemetery | 314-801-0764 |

## Before you fax

- Check the tables on page 3 of the SF-180: they say where the record is kept, by branch and separation date. Most are at the NPRC in St. Louis.
- Sign and date the request. Federal law requires written requests for non-archival records to be signed and dated within the last year.
- Send a separate SF-180 for each veteran; one fax can carry several.
- For an emergency, write it in the "Purpose" section and fax to 314-801-0764. For a burial outside a National Cemetery, include the next of kin's signature and proof of death.
- Don't send original documents: anything sent with the request is not returned.
- You can also order online with eVetRecs, or mail the form to National Personnel Records Center, 1 Archives Drive, St. Louis, MO 63138.

## Steps

1. Fill in the SF-180, then sign and date it.
2. Scan or photograph every page.
3. Enter 314-801-9195, or 314-801-0764 for an emergency, and add the pages.
4. Send, and keep the delivery receipt.
