# Fax an Aetna appeal or complaint

For an Aetna member complaint or appeal that is not a Medicare plan, fax Aetna's Member Complaint and Appeal Form to Aetna's National Fax Number, 859-425-3379. Aetna Medicare appeals and grievances use different numbers, listed below by type.

## Before you fax

- Aetna's Medicare appeal page asks you to choose your state, county and plan to get your form and, for expedited appeals, your own phone and fax number. If that page shows a different number for your plan, use the one on the page.
- The 859-425-3379 number is printed on Aetna's Member Complaint and Appeal Form (medical or dental). Aetna's page for non-Medicare complaints does not print a fax number itself; it says to mail or fax the form.
- Aetna also takes non-Medicare complaints online and by phone at the number on your member ID card. Medicare appeals can be filed online, by phone or by mail.
- Mail for non-Medicare complaints goes to Aetna, PO Box 14463, Lexington, KY 40512. Medicare appeal and grievance mailing addresses differ by type; they are on Aetna's Medicare appeal and grievance pages.
- If someone else files for the member, Aetna's form asks for a completed Authorized Representative Form when the person is a spouse, a child 18 or older, or another relative.
- Aetna says expedited Medicare appeals are decided within 72 hours, other authorization appeals within 30 calendar days, claim denials within 60 days and drug redeterminations within 7 days. Expedited decisions are not available for claim denials. Check your denial letter for your own filing deadline.
- If your hospital, home health, skilled nursing or rehabilitation care is ending, a fast-track appeal goes to the Quality Improvement Organization named on your Notice of Medicare Non-Coverage, not to Aetna.

## Steps

1. Pick the form: Aetna's Member Complaint and Appeal Form for a non-Medicare plan, or the Medicare form for your plan type from Aetna's appeal or grievance page. Fill it in and sign it.
2. Add bills, letters from Aetna and anything that supports your case, and scan or photograph every page.
3. Enter the number from the list above that matches your plan and what you are filing.
4. Send, and keep the delivery receipt as proof of the date you sent it.
